Application form (Form IS3, available from TISI web site) - the Client must have the orginal application form for submittal
Supporting documents are:
- One set of photographs of the product
- Brief description of manufacturing process
- List of equipment and instruments to be used in the manufacturing of the porduc
- Chart or details showing the process of quality control of the production
- Details showinig the process of examining the quality of product
- Lists of equipment and instruments to be used in examining the quality of product
- Certificate or copy thereof issued by the office of partnership and company in case of juristic person, or the commercial registrations in case of non- juristic person
- License to operate a factory
- Trade mark certificate (incase the licensee is the owner of that trade mark)
- Sample: 2 samples required for testing.
